    Abstract

    Molnipiravir is a promising RNA-Dependent RNA-Polymerase enzyme inhibitor that has started to be used in the treatment of SARS-COV-2. I present the first case who was infected with the Omicron variant and developed acute renal failure due to the use of molnipiravir.67 Year-old male patient with the diagnosis of hypertension and chronic obstructive pulmonary disease developed severe nausea, vomiting and diarrhea with the use of drugs. In the patient's examinations, severe deterioration in kidney functions occurred. The patient who developed acute tubular necrosis was taken to hemodialysis twice. IV hydration and supportive treatments were started. In the follow-up, clinical and renal functions improved. In patients over 65 years of age and with comorbidities, the adverse effects of molnipiravir should be considered and followed closely.

    The study was carried out during an emergency effort by the University to contain an Omicron outbreak in February, 2022. Medical personnel, students, and professors (1,039 individuals, aged 18-60) were quarantined in dormitories after possible COVID-19 exposure. Individuals were included in the current analysis if they had confirmed high-risk exposure to a COVID-19 infected roommate (defined as being within one to two meters of the infected individual for at least five minutes without a mask). NONS was offered to staff and students who reported exposure.


    After exclusion for being low-risk or testing positive via antigen test kit (ATK) within 24 hours, 625 student participants were included in the analysis. Of these, 203 participants used NONS at least four times per day and 422 volunteers did not use NONS. All volunteers were tested via ATK on the fifth, seventh, and tenth day of quarantine, and positive tests were confirmed by PCR.


    Among the 203 participants who used NONS, 13 tested positive (6.4% infection rate). Of the 422 in the control group, 108 participants tested positive (25.6% infection rate), a statistically significant difference from the treatment group (P<0.0001). Participants in the NONS group reported only a mild side effect of nasal irritation (8 of the 203, or 3.9%).

    A fox kit from Anoka County has tested positive for a deadly, highly contagious bird flu that has killed countless wild birds this spring to the concern of wildlife specialists.


    The positive case is the first confirmed of highly pathogenic avian influenza, or HPAI, in a wild mammal in Minnesota, according to the Department of Natural Resources.

    Abstract

    Severe acute respiratory syndrome coronavirus 2 is a new, highly pathogenic virus that has recently elicited a global pandemic called the 2019 coronavirus disease (COVID-19). COVID-19 is characterized by significant immune dysfunction, which is caused by strong but unregulated innate immunity with depressed adaptive immunity. Reduced and delayed responses to interferons (IFN-I/IFN-III) can increase the synthesis of proinflammatory cytokines and extensive immune cell infiltration into the airways, leading to pulmonary disease. The development of effective treatments for severe COVID-19 patients relies on our knowledge of the pathophysiological components of this imbalanced innate immune response. Strategies to address innate response factors will be essential. Significant efforts are currently underway to develop vaccines against SARS-CoV-2. COVID-19 vaccines, such as inactivated DNA, mRNA, and protein subunit vaccines, have already been applied in clinical use. Various vaccines display different levels of effectiveness, and it is important to continue to optimize and update their composition in order to increase their effectiveness. However, due to the continuous emergence of variant viruses, improving the immunity of the general public may also increase the effectiveness of the vaccines. Many observational studies have demonstrated that serum levels of vitamin D are inversely correlated with the incidence or severity of COVID-19. Extensive evidence has shown that vitamin D supplementation could be vital in mitigating the progression of COVID-19 to reduce its severity. Vitamin D defends against SARS-CoV-2 through a complex mechanism through interactions between the modulation of innate and adaptive immune reactions, ACE2 expression, and inhibition of the renin-angiotensin system (RAS). However, it remains unclear whether Vit-D also plays an important role in the effectiveness of different COVID-19 vaccines. Based on analysis of the molecular mechanism involved, we speculated that vit-D, via various immune signaling pathways, plays a complementary role in the development of vaccine efficacy.

    Let me see. My uncle got shingles within a week of his second shot, and was hospitalized with pulmonary edema about three days after his third shot. (His brother, my father in law, passed away three days after his fourth dose.) But! I'm happy to report that my uncle is fine after his fourth shot two weeks ago. My hardy wife was totally incapacitated for over a day after her second dose. One of my kids had large lumps under his right armpit for 2 months after his second dose. A few months ago I bumped into a 65ish year old slim fit guy I hadn't seen in years. He shared the crazy news of how he had a heart attack in the last year, and his wife a stroke. (She is now in a care home because of it.) I asked him if they had been vaccinated, then I asked him if his heart attack and her stroke had occurred soon after vaccination. It hadn't even occurred to him, and he was a bit rattled to compute they had both occurred within a few weeks after their vaccinations. Then there was my neighbour's friend who lost vision in one eye after vaccination, and another friend of the same neighbour who lost his hearing in one ear after vaccination.

    This is off the top of my head. But my current social circle is pretty small, so no doubt I'm missing out on a lot of stuff.
