    Coronavirus missteps from CDC and FDA worry health experts

    "If you don't trust the agencies that are telling you to do this, then you don't have your key weapons to fight against a pandemic," one doctor said.

    Dr. Stephen Hahn, the embattled head of the Food and Drug Administration, offered an assurance Monday: Any vaccine for public use will be approved "on the basis of science and data."


    "We will not make that decision on the basis of politics," he said in an interview with "CBS Evening News." "That's a promise."


    Hahn made the pledge after a series of recent public missteps involving the FDA and the Centers for Disease Control and Prevention — two of the federal agencies critical to the U.S. coronavirus response — which have damaged their reputations at a time when they are needed the most, according to seven prominent doctors and scientists who spoke to NBC News. They say that the recent events are clear signs of political interference from the White House and that they have shaken their trust and confidence in the leadership of the agencies.


    "It's an enormous scandal," said Carl Bergstrom, a biologist at the University of Washington who has become an outspoken critic of the U.S. pandemic response and has written extensively about misleading health information. "What it looks like at this point is you have a White House altering public health advice to improve election chances to the detriment of American lives."


    In an interview published Sunday in The Financial Times, Hahn said the FDA could fast-track a coronavirus vaccine by issuing an emergency use authorization before the end of Phase 3 clinical trials. The comments were met with an outcry from public health experts, prompting him to clarify in the CBS interview that a vaccine wouldn't be politicized.


    Before that, Hahn misrepresented data about convalescent plasma, leading him to apologize for having overstated the potential treatment's benefits. He also ousted the agency's chief spokesperson over the fiasco, The New York Times reported.

    The CDC sparked its own crisis when it inexplicably changed its guidance on COVID-19 testing in a way that ran contrary to the best available scientific evidence, Bergstrom said. The updated recommendations, posted on the agency's website Aug. 24, suggested that people exposed to the coronavirus "do not necessarily need a test" unless they exhibit symptoms, are older or have existing medical needs that make them especially vulnerable to the virus.


    But it has been known that infected people can be contagious before they experience symptoms and that people can also spread the virus even if they remain asymptomatic.


    The CDC's director, Dr. Robert Redfield, backtracked and issued a statement saying that "all close contacts of confirmed or probable COVID-19 patients" may consider getting tested.


    Loren Lipworth, an epidemiologist at Vanderbilt University Medical Center in Nashville, Tennessee, said the recent developments are troubling because they undercut the CDC's mission.


    "As epidemiologists, the CDC has always been who we turn to for guidance and for the data," she said.


    But the agency's handling of the pandemic may be jeopardizing that legacy. In an opinion piece for The New York Times, Harold Varmus, a former director of the National Institutes of Health, and Rajiv Shah, president of the Rockefeller Foundation, called the agency "dysfunctional" and argued that it may be time to ignore the CDC's altered guidelines.


    "Now ... it’s up to states, other institutions and individuals to act," they wrote.


    Bergstrom said anything that compromises clear public health messaging could be very damaging, especially because scientists learn new things about a virus and its effects on humans as a pandemic evolves. About eight months into the worst pandemic in more than a century, research has shown that staving off the coronavirus relies on three pillars that work in concert: testing widely, social distancing and wearing masks.


    "If you don't trust the agencies that are telling you to do this, then you don't have your key weapons to fight against a pandemic," he said.


    With the FDA commissioner's blunder, Lipworth said, it's particularly important for the person leading the agency responsible for overseeing the development of potential coronavirus treatments and vaccines to convey information correctly.The situation has only furthered concerns that the agencies have been politicized. The spokesperson who was let go from the FDA was previously a reporter for One America News Network, a far-right media outlet that has been a strong supporter of President Donald Trump.


    Beyond the obvious dangers of mischaracterizing data about a potential treatment, Lipworth said, it's still too early to know whether convalescent plasma is, in fact, beneficial. While the therapy has been shown to be safe, clinical trials to test its effectiveness continue, including at Vanderbilt University.


    "The evidence is certainly not conclusive whether or not Dr. Hahn communicated it correctly," she said. "But even if he had, we're still not at a point where we can say there's conclusive evidence of the benefits."


    All of the doctors and public health experts who spoke to NBC News expressed concern that Hahn's misrepresentation could tarnish the FDA's reputation. The erosion of trust could be especially problematic at the FDA, the agency responsible for evaluating the safety and effectiveness of vaccine candidates, said Dr. Stanley Perlman, a professor of microbiology and immunology at the University of Iowa.


    "It's a real worry, especially when we have people who don't trust the government and don't want vaccines and barely believe that this virus is causing a problem," he said.


    Dr. Steven Goodman, a professor of epidemiology and medicine at Stanford University, said the recent incidents may cause people to question the motives of the federal agencies that respond to public health emergencies.


    It reduces confidence that they are without political influence," he said. "Any statement that the FDA makes that they have to amend or qualify, or any hint that they are lowering their standards, makes future statements more suspect in the eyes of the public."


    And while public health has been shaped by politics throughout history, the recent events raise a different sort of red flag, said Dr. Carlos del Rio, executive associate dean of the Emory University School of Medicine in Atlanta, who is an investigator for Moderna's coronavirus vaccine clinical trials.


    "Politics has always played a role in public health — think about HIV, for example — but it should not be partisan," del Rio said. "It should never favor one party over another."


    The missteps add frustration for scientists and public health experts who are already operating in a politically charged environment rife with misinformation.


    "I've been concerned before, but this elevates it to angry," Goodman said.


    Dr. David Dowdy, an associate professor of infectious disease epidemiology at the Johns Hopkins Bloomberg School of Public Health, called the recent events a "tremendous blow" to the credibility of the CDC and the FDA. He said he hopes the damage isn't permanent.


    "Long term, I hope that this is not something that will tarnish them too greatly if and when we have a political situation where science is not a political issue anymore," he said.

Yang et al. analyzed blood samples from young children and adults, as well as tissues from deceased organ donors, characterizing the B cell receptor (BCR) repertoires specific to six common pathogens and two viruses that they had not seen before: Ebola virus and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Children had higher frequencies of B cells with convergent BCR heavy chains against previously encountered pathogens and higher frequencies of class-switched convergent B cell clones against SARS-CoV-2 and related coronaviruses.     Abstract

    Vaccination and infection promote the formation, tissue distribution, and clonal evolution of B cells, which encode humoral immune memory. We evaluated pediatric and adult blood and deceased adult organ donor tissues to determine convergent antigen-specific antibody genes of similar sequences shared between individuals. B cell memory varied for different pathogens. Polysaccharide antigen–specific clones were not exclusive to the spleen. Adults had higher clone frequencies and greater class switching in lymphoid tissues than blood, while pediatric blood had abundant class-switched convergent clones. Consistent with reported serology, prepandemic children had class-switched convergent clones to severe acute respiratory syndrome coronavirus 2 with weak cross-reactivity to other coronaviruses, while adult blood or tissues showed few such clones. These results highlight the prominence of early childhood B cell clonal expansions and cross-reactivity for future responses to novel pathogens.

    After months of sputtering vacillation, the CDC says fully vaccinated Americans can ditch masks and don't have to social distance in most indoor places. Unless you’re in a room with Andrew Cuomo. In that case - run! Speaking of that ass - even after restrictions have been lifted, he's still being cautious, claiming to rely "on the facts and the science." Yeah now's he careful. Was he relying on the facts and the science when he sent COVID patients to rest homes? If that were facts and science, then his treatment of women were delightful acts of chivalry.


    So the CDC says you can skip wearing masks outside. In other news, the CDC says women can vote and wear pants. (Still - not sure I’m for either) According to the New York Times, the CDC is "finally catching up to the science." Not true. They're finally catching up to you.


    We always knew that COVID doesn't spread outdoors. It's not like chlamydia at Coachella. Which I learned, sadly. We also knew that vaccination was supposed to eliminate risk of spread, which was motivation for getting it. The CDC however - so obsessed with the slightest risks - the kinds that are smaller than choking to death on soup - made their guidance as helpful as financial advice from Bernie Madoff. So why did they change?


    Is it cuz they now followed the science? No, because if they had followed the science they would have come to this conclusion a while ago. Fact is, they didn't follow the science. They followed you. They knew that if they kept pushing the American public too far, we were gunna push back. Plus the experts knew you were right, but they couldn't admit it. (Anybody married is laughing at that right now.)


    See, all their guidance is based on fear. Fear of not being wrong, but fear of being exposed. Why is that? Well, these are not leaders. How can you tell? They played it safe. That's not a leader. That’s your 75-year-old Aunt Tilly backing out of her driveway. Leaders take risks. They listen to experts, but then weigh the risks against the benefits. It’s like a coach who talks to his assistants - and then goes for it on 4th down. That's a hockey reference, Kat.


    A leader must face obstacles head-on and if they fail, be willing to take the fall. As for the experts in the media - their constant covering of their asses created an ever-worsening feedback loop. A weak politician might accept the risks of reopening - but retreat the moment someone tweets, "but people will die!" So they told you to sit tight at home, masked. Even if you're living with older folks.


    The irony: the people who yell "wear a mask or you’re killing grandma" were supporters of the biggest grandma killers of all. And so after every time an adult publicly assessed risk - you'd get some media asshat asking "how many people are you willing to see die?"


    Just over a month ago CDC director Rochelle Walensky said she had a sense of "impending doom." Thanks for the pep talk, doc - you must be great on a plane flight. Fact is we accept death, every day. Sad but true. Insurance actuaries count the future deaths we naturally accept. No one has ever been able to escape the phenomenon of death. Except Keith Richards.


    Leaders get that, but experts don’t. Experts aren’t paid to lead. Like gramps taking a leak at 3 AM. They’re just paid to spray their opinions all over the place. And the American people aren't experts. They're better.

    Think about our ancestors who came here - the opposite of a safe space. No expert was advising them to come here. Like marshmallow-topped sweet potatoes, it's unique to America. We're number one, but because we won't take number 2 from anyone.

    Taking reasonable precautions but remaining skeptical of what all the purportedly "best experts" here in the United States are telling us about every aspect of their belated crisis management and response (especially on their pimping of vaccine development to prevent the disease).


    Here are some plain, nonhysterical facts: The U.S. Centers for Disease Control is a bloated federal government agency with a long history of incompetence, fraud, secrecy, mission creep and shady alliances with both social justice causes on the left and private corporations on the big business right. The "deep state" of entrenched bureaucrats embedded in the Beltway bowels is alive and thriving at the CDC. The unelected elites who've occupied top offices at the public health-industrial complex are hostile to public scrutiny while clamoring for ever-ballooning budgets. Their recommendations have often been detrimental to citizens' well-being and freedom.


    And, yes, many of the scientists who work there are rabidly anti-conservative and anti-Trump.

    It's a brazen lie that President Donald Trump "slashed" CDC funding, which is being used as an excuse for the agency's coronavirus unpreparedness. Cognitively impaired and truth-challenged Joe Biden made the claim during last week's Democratic debate that these purported "Draconian cuts" put Americans at risk. But Trump's budget proposal to cut some sliver of CDC fat has never been enacted, and Congress increased funding, instead.


    At nearly $7 billion, CDC's annual budget is more than 200% larger than it was two decades ago. On top of that, a shady big business lobbying group called Corporate Friends of CDC has raised hundreds of millions of supplemental dollars -- which raises serious conflict-of-interest issues. In 2007, Sen. Tom Coburn's fiscal audit of the agency discovered vulgar expenditures including CDC syphilis prevention funds spent to host a "safe-sex" event with a porn star, CDC HIV/AIDS prevention funds spent on a transgender beauty pageant, and $45 million in CDC funding spent on conferences featuring prostitutes, protests and beach parties.


    Despite the flood of money, the agency been caught flat-footed on outbreak after outbreak. They squander untold millions on other health threats in favor of pushing gun control and nanny state hobby horses (TV violence, helmet laws, video games, anti-bullying campaigns and explicit sex education, for example). CDC has one primary job -- disease control -- but has managed to botch it without ever learning from past failures. Let me remind you of some of the CDC's long history of royal screw-ups:


    --The notorious 1976 swine flu vaccine scandal was catalyzed by mass hysteria whipped up by CDC junk scientists clamoring for more money; Congress obliged and nearly 45 million Americans were unnecessarily jabbed with a vaccine for a disease that had fizzled by the time the shots were ready. The vaccine resulted in an increased risk of developing Guillain-Barre syndrome -- leading to muscle weakness and paralysis.


    --In 1999, the CDC misspent $22.7 million appropriated for chronic fatigue syndrome and was investigated in 2001 for squandering $13 million on hepatitis C research.


    --In 2000, the agency essentially lied to Congress about how it spent up to $7.5 million earmarked each year since 1993 for research on the deadly hantavirus. "Instead, apparently without asking Congress, the CDC spent much of the money on other programs that the agency thought needed the funds more," The Washington Post disclosed at the time. The diversions were impossible to trace because of shoddy CDC bookkeeping practices.


    --In 2009, the CDC recalled 800,000 doses of swine flu vaccine for children ages 6 months to 3 years after the products manufactured by Sanofi Pasteur failed potency tests.


    --In 2012, the CDC's main building housing infectious anthrax, SARS and monkeypox gases was discovered to have been leaking deadly pathogens due to a flawed engineered airflow system.


    --In 2014, the CDC's lackadaisical response to Ebola virus gave the greenlight to Patient Zero Thomas Eric Duncan and infected medical personnel to board commercial airlines while ill -- exposing untold numbers of travelers to the deadly disease. Expensive isolation chambers that had been subsidized by taxpayers to the tune of $15 million sat buried in a Georgia warehouse.


    --For the past few years, the agency has employed a fear campaign to induce Americans to get regular flu shots (formulated based on guesswork) using flawed statistics and despite international scientific conclusions that there is "no evidence" to support the CDC's assumptions that the vaccine reduces transmission of the virus or the risk of potentially deadly complications.


    --Just this week, the Informed Consent Action Network reported astonishing results from its lawsuit against the CDC to obtain scientific documentation for the agency's claim that "vaccines do not cause autism" -- specifically for the first seven vaccines given to your child in their first six months of life. CDC had stonewalled, and then finally produced 20 studies -- 18 of which were irrelevant to ICAN's public records request and two of which suggest that vaccines may indeed cause autism. In other words: The CDC is lying while continuing to smear vaccine critics as "conspiracy theorists" and public health threats.


    As if to underscore my point about this agency's misdirected priorities and rank political pandering, the CDC director this week found it more urgent to condemn Republicans as racist for accurately describing the origins of the virus from Wuhan, China, than to do his job.


    Informed diagnosis: The "best people" -- who have now been rewarded with more than $8.3 billion in new "emergency funding" -- are not in charge.

  • Hormone Drugs May Disarm Coronavirus Spike Protein and Stop COVID-19 Disease Progression

    A new Penn Medicine study shows how anti-androgen drugs disrupt key receptors required for viral invasion of cells.


    Hormone drugs that reduce androgen levels may help disarm the coronavirus spike protein used to infect cells and stop the progression of severe COVID-19 disease, suggests a new preclinical study from researchers in the Abramson Cancer Center at the University of Pennsylvania and published online in Cell Press’s iScience.


    Researchers show how two receptors — known as ACE2 and TMPRSS2 — are regulated by the androgen hormone and used by SARS-CoV-2 to gain entry into host cells. Blocking the receptors with the clinically proven inhibitor Camostat and other anti-androgen therapies prevented viral entry and replication, they also showed in lab studies.


    The findings provide more insight into the molecular mechanisms of the virus but also support the use of anti-androgen therapies to treat COVID-19 infections, which are currently being investigated in clinical trials and have produced promising results. They also support data showing increased mortality and severity of disease among men compared to women, who have much lower levels of androgen.


    “We provide the first evidence that not only TMPRSS2, which is known to be regulated by androgen, but ACE2 can also be directly regulated by this hormone,” said senior author Irfan A. Asangani, PhD, an assistant professor of Cancer Biology in the Perelman School of Medicine at the University of Pennsylvania. “We also show that the SARS-CoV-2 spike relies on these two receptors to impale and enter cells, and that they can be blocked with existing drugs. That’s important because if you stop viral entry, you reduce the viral load and disease progression.”

    Camostat is a drug approved for use in Japan to treat pancreatitis that inhibits TMPRSS2. Other anti-androgen therapies, including androgen deprivation therapy used to treat prostate cancer, serve similar functions.


    Driven by the disparity in COVID-19 rates between men and women, the cancer researchers sought to better understand the role androgen and its receptors played in infections, which has long been known to be a driver of prostate cancer.


    The researchers performed experiments with a pseudotype SARS-CoV-2, which carries the spike proteins of the virus but not its genome.


    In mice with significantly reduced androgen levels and cells treated with anti-androgen treatments, the researchers found little to no expression of TMPRSS2 and ACE2, suggesting both are regulated by the hormone. They also observed how inhibiting TMPRSS2 with Camostat blocked priming of the spike for entry into cells. That drug, as well as enzalutamide, an anti-androgen therapy used to treat prostate cancer, also blocked the virus’ entry into lung and prostate cells. Combining these therapies, they found, significantly reduced virus entry into cells.


    “Together, our data provide a strong rationale for clinical evaluations of TMPRSS2 inhibitors, androgen-deprivation therapy / androgen receptor antagonists alone or in combination with antiviral drugs as early as clinically possible to prevent COVID-19 progression,” the authors wrote.


    In March, researchers from Brazil reported preliminary results of 600 hospitalized patients in a clinical trial investigating proxalutamide, a new anti-androgen therapy, for the treatment of COVID-19. The drug reduced mortality risk by 92 percent and shortened the median hospital stay by nine days versus the standard of care, the researchers reported.

    Next, Asangani and his colleagues will partner with Susan R. Weiss, PhD, a professor of Microbiology and co-director of the Penn Center for Research on Coronaviruses and Other Emerging Pathogens, to investigate the findings further using live SARS-CoV-2, as well as anti-androgen therapies’ ability to block different variants of the virus, which continue to emerge and are often differentiated by their spike proteins.

    Researchers at Amsterdam UMC have discovered why patients become seriously ill after being infected with the coronavirus. They identified that aberrant antibody responses are the main cause of becoming critically ill. In addition, they identified a drug that can counteract this derailed immune response, which may be used to treat seriously ill COVID patients in the ICU. This drug is already FDA approved for treatment of particular autoimmune diseases.


    Immunologist Den Dunnen explains: "During an infection, our immune system produces antibodies. Normally, these antibodies protect your body against viruses. This also holds true for most people that become infected with the Corona virus. But in COVID-19 patients that become seriously ill, the antibody response is derailed. These patients do not only make extremely high amounts of antibodies against the virus, but these antibodies also have an aberrant structure. This causes an extreme inflammatory reaction in the lungs."

    De Winther, an expert on immune cells: "The antibodies that our bodies make are shaped like a Y. The top binds to the virus, while the tail binds to the immune cells in the lungs, thereby activating these cells. We now know that this tail is different in patients who are seriously ill. To put it simply, the sugars that are normally attached to antibodies are very different in these patients. The result is that the immune cells in the lungs are activated much too strong."

    The different composition of the antibody's tail has three effects, Den Dunnen explains. "The immune system becomes completely over-activated, which is sometimes referred to as a cytokine storm. The inflammatory response that was meant to attack the virus now destroys the patient's own tissues. In addition, the blood vessels become leaky, the lungs fill up, and platelets start to clump together. Admission to the ICU is often unavoidable and the prognosis for these patients is generally not good."



    The Amsterdam UMC has a lot of immunological expertise, and with that background knowledge in mind drugs were selected that specifically inhibit the inflammatory response caused by the abnormal antibodies, while leaving the rest of the immune system intact. "The drug we studied, fostamatinib, ensures that the immune cells in the lungs no longer react to the abnormal antibodies, but still react to the virus. This specificity makes this drug a promising candidate for treating seriously ill COVID patients."
