According to Vietnamese media, the Vietnamese health ministry added 400 mg Molnupiravir to its COVID-19 treatment protocol, which also includes Favipiravir and Remdesivir. TrialSite reported that in Ho Chi Minh City, the department of health has conducted an unorthodox home care pilot, including the experimental antiviral drug still under investigation. The pilot care program targets COVID-19 patients with mild symptoms aged 18 to 65 with no signs of pneumonia or oxygen shortage for a five-day regimen. TrialSite reported on this program, including an interview with a participant. But the home care program involving the Merck drug was part of a local municipal effort. However, based on a recent media entry, the Vietnam Ministry of Health at the national level includes the still-experimental drug as part of its national treatment protocol. Is Molnupiravir on an even faster track in Vietnam than in the lucrative American market?

TrialSite wasn’t able to verify yet with the Vietnam Ministry of Health as to the veracity of the Vietnam Net piece, however, the timing coincides with Merck’s recent report that due to positive data, a large study’s independent safety data monitoring committee stopped the study. The company has announced its planning on submitting for emergency use authorization with the U.S. Food and Drug Administration (FDA) and other national regulatory agencies imminently.

An Indian firm provided the drug supply for the initial study—Merck, announced partnerships with multiple generic pharmaceutical companies in India to produce and distribute the drug in the region. Initially proposed for mild-to-moderate cases, a recent report in India indicates the study drug may not be effective in moderate cases. This could make sense given later stages of the SARS-CoV-2 disease are more inflammatory in nature.

Because of safety concerns, the Vietnamese news reminded the reader that the drug is “not for pregnant women or those who are planning to get pregnant.”

Favipiravir & Remdesivir

Meanwhile, a competitive oral antiviral is accepted in this southeast Asian nation of 97.4 million people. Pregnant women can’t use Favipiravir—or those breastfeeding or those under 18 and people with serious liver or kidney failures. Called Avigan, the drug is produced by FUJIFILM Toyama Chemical in Japan. TrialSite has extensively chronicled the use of this drug as an antiviral targeting COVID-19. Dozens of nations have approved the use of this drug for mild cases of COVID-19, including Russia, India, and Thailand.

The Chinese military successfully secured a patent for the drug in that country while Vietnam media reports an effort to shorten the synthesis process of the drug at the Vietnam Academy of Science and Technology (VAST) was successful.

With a similar mechanism of action to that of Remdesivir, it can purportedly stop the replication of the coronavirus.

Remdesivir represents the third antiviral drug accepted by the Ministry of Health in Vietnam. In Vietnam, Remdesivir is used on COVID-19 patients presenting symptoms less than 10 days experiencing respiratory failure necessitating oxygen, high-flow oxygenation (HFNC), or non-invasive mechanical ventilation reports Vietnam-Net.

Conclusion

If this particular Vietnam media is correct, Molnupiravir has been formally accepted nationwide as an antiviral regimen for mild COVID-19. The experimental drug has definitely been included in an unconventional home care program as it has not been approved or authorized on an emergency basis anywhere.

And that furthermore in some senses infection rate is a bad measure of success, because we reckon with delta everyone will catch it at some point - it is just that if they are vaccinated first they have lower mortality? So a lower infection rate now juts means you have higher some other time.

I said in some senses because treatment is better now than it was 12 months ago, and will probably be a lot better in 12 months than it is now.

The UK's high rate throughout Summer actually gives me a lot of hope that we will avoid a winter surge.

Look at the details. infections now are very high (1 in 15) amongst secondary school children (who we have started vaccinating but it is slow and in any case take-up is only about 50%). Those children push COVID into the rest of the community, but a few more months and we should have enough covid-infection immunity + a bit more vaccination - vaccination is much more efficacious in kids - and that infection driver will stop.

I expect that the UK pandemic soon will end for people age < 50 . Except they do vaccinate to many children...

The problem is the group 75+ where vaccines only delay the infection or have no effect as for age 80+.

UK should learn from India. But may be the ruling "blue blood" arrogance will not allow it.

It seems unknown how large might have been the “recovered group”….The author complains about the fact that there doesn’t seem to be an interest by public media, to find out how many of the infected were vaccinated and how many not. From the article:

Again, not mentioned in official media (see link in the report), that 2G (Geimpft=double vaxx + Genesen=recovered) ended up being just 1G…all double vaxx only….

I understand that you talk of paranoia. I so far I could not find an official confirmation for all 86. The responsible officials stopped all communication after the first 52 have been tested. Obviously and officially the first 52 were all double vaxx.

This is no surprise as even worst case studies show a 13x better protection and extreme cases show 1000x for recovered.

Key is: With this you can no longer sell vaccines to recovered. You also have to offer antibody tests for infections. Even worse if you hand out vaccine passports then recovered should get a stamp for 10 years and the pitiable Pfizer victims should get one for 3..6 months only depending on age. Moderna so far should get at least 2 years.

It's all politics what today is = "about money". Even worse in Germany (as all western & eastern countries) all parties are involved.

So clever people help themselves!

So it is still not clear whether obesity alone is a risk! Could also be a chance for some!

So, when comparing different immune response persistence times it is more about the human immune response (to that pathogen) than the vaccine.

While it is true that the very specific vaccines used for 1st gen COVID could be more likely to have short persistance, because they expose the body to fewer proteins, but Flu vaccines, for example - surely a nearer fit than the ones you mention, last only 6 months. Also worth noting that the N protein response to COVID (N IgG) - from so-called natural infection - reduces rapidly over 6 months too.
