    Abstract:

    The current COVID-19 pandemic is caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV2) and has an enormous impact on human health and economy.

    In search for therapeutic options, researchers have proposed resveratrol, a food supplement with known antiviral, anti-inflammatory and anti-oxidant properties as an advantageous antiviral therapy for SARS-CoV-2 infection.

    Here, we provide evidence that both resveratrol and its metabolically more stable structural analog, pterostilbene, exhibits potent antiviral properties against SARS-CoV-2 in vitro. Resveratrol and pterostilbene showed antiviral activity in African green monkey kidney cells and in human primary bronchial epithelial cells cultured in an air-liquid interface system. Mechanistic analyses demonstrated that both compounds actively interfere with the post-entry steps of virus replication cycle and their antiviral activity is long-lasting. Collectively, our data indicate that resveratrol and pterostilbene are promising antiviral compounds to treat SARS-CoV-2 infection and advocate

    evaluation of these compounds in clinical trials.

    Lou how do these guys know their preferred treatments are effective? Seems to me without some comparison that is unbiassed (= RCT) they cannot have much evidence? I mean, almost all (99%) of early COVID patients recover. That percentage changes by 10X (e.g. 0.3% to 3%) for a change in median age from 30 to 50 etc. many other effects change it too. I'm sure you know that mortality figures vary wildly according to how many people getting milder versions of the disease are counted, how many are treated in hospital (for the hospital mortality figures), etc.

    This sounds like reasonable evidence to me:

    Overall mortality rate of patients treated according to IDEA protocol was 0.59 % (1 death in 167 treated cases).

    As a comparison, estimated overall mortality rate in Argentina is approximately 2.1 % (official data by September 2nd, 2020).

    Regarding moderate to severe cases, i.e. patients needing hospitalization, only 1 patient out of 32 receiving IDEA treatment

    died (3.1 %), whereas mortality rate published in articles from Spain, Italy and Spain is ca. 25 %. Moreover, a group of 12

    patients were hospitalized in Eurnekian hospital in the same period but did not receive IDEA treatment. Three of them died,

    thus presenting a mortality rate of 25 %, i.e. significantly higher than that of those receiving IDEA treatment."


    Also note that the moderate-to-severe average age was nearly 60, and that they were using oxygen or ventilated.

    Let's leave the doctor-patient team decide the best course, but who would want to be on the

    placebo arm of an RCT? However, if they do, then they should be allowed.

    Actually it does not quite do that. The devil as always lies in the detail.

    It shows, with high statistical significance, that HCQ + vitamin D is more effective than HCQ on its own.

    Anyone reading the paper easily sees that several (less plausible) hypotheses are possible.

    These were patients fairly far along in the disease, and HCQ alone probably would not help,

    but it appears that HCQ is a standard protocol for this medical group - could it be that they

    really believe it's a placebo? Certainly it is possible that Vit-D is solely responsible (but, I would

    bet against it)- easily checked since CRTs are in process. Synergy seems more probable.


    Also, since you thought the Recovery study was legitimate despite mega-dosing seriously infected

    patients with HCQ, you may want to do a web search on recent papers on the prevalence of

    Covid liver injury - which, likely exacerbated the toxicity.
