    A UK study has found that Merck & Co’s oral COVID-19 therapy Lagevrio was unable to reduce hospitalisations compared to placebo in patients at higher risk from the virus, adding fuel to assertions in some quarters that its authorisation was premature.


    The 25,000-subject PANORAMIC trial conducted by researchers at Oxford University looked at the addition of a five-day course of Lagevrio (molnupiravir) to standard care in people aged over 50, or adults aged 18 and over with conditions that raised their risk of severe COVID-19, comparing it to standard care plus a placebo on all-cause hospitalisation/death within 28 days.


    Preliminary results show that, while Lagevrio hastened the time to recovery from COVID-19 by around six days, it was no better than placebo at keeping patients out of hospital.


    Reducing hospitalisations was one of the main drivers for rapid adoption of the drug – which made more than $4 billion in sales in the first half of this year – as it was hoped that it could help alleviate the pressure on healthcare systems stretched to breaking point by the pandemic.


    The results are at odds with Merck and partner Ridgeback Therapeutics’ much smaller MOVE-OUT trial, which found a reduction in the risk of hospitalisation or death in high-risk patients by around 30%, having shown a 48% cut at the interim stage. Although, unlike PANORAMIC, that excluded patients who had been vaccinated against SARS-CoV-2.


    One key finding in PANORAMIC was a much lower rate of hospitalisations than predicted, which ties in with the observation that the now-dominant Omicron strain of the virus is less severe than earlier variants. The organisers of the study have now turned their attention to Pfizer’s oral antiviral Paxlovid (nirmatrelvir/ritonavir), with results expected in the coming months.

    Nearly half the country (44%), or about 114 million Americans, give poor (30%) or failing (14%) grades to the U.S. healthcare system, percentages that climb higher and grow even more negative when it comes to affordability and health equity, according to a new report from West Health and Gallup, the polling organization.


    The 2022 West Health-Gallup Healthcare in America Report asked a nationally representative sample of more than 5,500 Americans to provide a letter grade (A-excellent, B-good, C-satisfactory, D-poor and F-fail) for the healthcare system overall and to give individual grades for affordability, equity, accessibility and quality of care.


    High marks were in short supply across the board, with the healthcare system getting an average grade of C-minus. Women and Hispanic and Asian Americans were more negative, with about half of each group assigning it a grade of D or F compared to about 40% of males, and 43% of White and Black Americans.


    Nothing, however, earned more failing grades than affordability, which for three-quarters of Americans -; an estimated 190 million adults -; deserved no higher than a D (41%) or F (33%), for an average grade of D-minus. A top grade of A was virtually nonexistent (1%), only 6% went as high as a B, and 19% gave it a middling grade of C. The negative feelings about healthcare affordability were strikingly similar across gender, age, race, household income and political persuasion.

    Why do at least some SARS-CoV-2 infections remain asymptomatic? Led by prominent University of California, San Francisco (UCSF) immunologist Jill Hollenbach, Ph.D., MPH, who also has specialized in the population genetics of the human leukocyte antigen (HLA) system, the San Francisco-based team hypothesized that it is in fact, the variation in an individual’s HLA system that drives the differences in COVID-19 symptomatic presentation.


    Uploaded recently on the preprint server medRxiv, the study results will be peer-reviewed and published, then making the evidence weightier.


    What is HLA?

    A complex of genes on chromosomes encoding cell surface proteins responsible for the regulation of the immune system, the cell surface proteins are known specifically as the major histocompatibility complex (MHC)—the mechanism of how cells communicate infection to the immune system.


    The Study

    Leveraging the National Marrow Donor Program as well as accessing data on subjects who had high-resolution HLA genotyping data, the study team employed the use of smartphone-based survey data to track Covid-19 symptoms and outcomes to associated symptoms with HLA loci.


    What were the Results?

    Hollenbach and team discovered a highly statistically significant association.


    HLA-B*15:01 was significantly increased in asymptomatic individuals in the discovery cohort compared to symptomatic (OR = 2.45; 95%CI 1.38-4.24, p = 0.0016, pcorr = 0.048), and they also reproduced this association in the replication cohort (OR= 2.32; 95%CI = 1.10-4.43, p = 0.017).


    The study team discovered robust association of HLA-B*15:01 in the combined dataset (OR=2.40 95% CI = 1.54-3.64; p = 5.67 x10-5) and observed that homozygosity of this allele increases more than eight times the chance of remaining asymptomatic after SARS-CoV-2 infection (OR = 8.58, 95%CI = 1.74-34.43, p = 0.003).


    Finally, they demonstrated the association of HLA-B*15:01 with asymptomatic SARS-Cov-2 infection is enhanced by the presence of HLA-DRB1*04:01.


    Their conclusion: HLA-B*15:01 is strongly associated with asymptomatic infection with SARS-CoV-2 and is likely to be involved in the mechanism underlying early viral clearance.

    Albert Bourla, Pfizer’s CEO was called to testify in front of the special committee but pulled out last minute.


    He was set to face tough questions from several MEPs regarding his secretly negotiated multi-billion-euro vaccine deal, conducted via private text messages and phone calls with the President of the European Commission, Ursula von der Leyen. It’s noteworthy that in Part 1 of my report, the leaked EMA emails revealed that Bourla also “lobbied” Peter Marks, director of the Center for Biologics Evaluation and Research (CBER) at the FDA.


    Janine Small, Pfizer’s President of International Developed Markets, represented the giant pharmaceutical company, who’s made billions from their “most successful product ever," instead of Bourla. She promised to get back to Rivasi’s critical questions formally in writing. I and indeed many others, look forward to reading Pfizer’s official response.

    A Massachusetts General Hospital physician-investigator led a case study involving the treatment of delayed localized necrotizing inflammatory myositis post-vaccination with the COVID-19 mRNA vaccine produced by Moderna known as mRNA-1273. Operating in the Division of Rheumatology, Allergy and Immunology, Jennifer Li, MD, and Seth Bloom, MD, Ph.D., found a patient had a serious adverse event in the form of a localized necrotized inflammatory myositis after the third dose of the mRNA-1273 mRNA vaccine.


    After receiving his third dose of the mRNA-1273 vaccine in his left upper arm, a 64-yeard old male two-and-a-half weeks thereafter experienced symptoms such as pain, swelling, and fevers that didn’t respond to a standard antibiotic regimen.


    The Condition

    A rare condition involving severe inflammation and death of tissues associated with muscles, Localized necrotizing inflammatory myositis can be triggered by autoimmune disorders but also injuries or infections.


    The Regimen

    The care team under the leadership of Dr. Li at Massachusetts General Hospital employed the use of MRI imaging and a biopsy to determine a diagnosis of localized necrotizing inflammatory myositis. A regimen involving prednisone was used successfully to alleviate the symptoms.


    Conclusion

    The Boston-based team treated the myositis successfully but they could not figure out the mechanism of action associated with the Moderna mRNA-based vaccine. Li and the team report that this is a topic involving more research.
